Privacy Policy Notice
As of June 8, 2001

Congress recently passed the Gramm-Leach-Bliley Act (GLBA), which deals in part with how we as a company treat nonpublic personal information.  We at Budd Agency, Inc. are particularly concerned about the privacy of our individual customers. Therefore, we are sending you this notice to help you understand how we collect information about you, the type of information we collect, and with whom that information may be shared.  In compliance with the GLBA and the laws of this state, we are providing you with this document, which notifies you of the privacy policies and practices of Budd Agency, Inc. 

Our Privacy Policies and Practices

I. Information we collect:

A. We may collect nonpublic personal information about you from the following:

-  information we receive from you on applications or other forms
-  information about your transactions with us or our affiliates
-  information we receive from a consumer reporting agency

B. Unless it is specifically stated otherwise in an amended Privacy Policy Notice, no additional information will be collected about you. 

C. We do not collect nonpublic personal information from anyone other than the individual(s) proposed for coverage.

D. According to the Fair Credit Reporting Act, we may obtain information about you from investigative consumer reports prepared by third parties at our request.  Upon request, you are entitled to receive a copy of the report.

II. Information we disclose:

We may disclose the information that we collect (as described above) about you without your permission to the following types of institutions and reasons described:
                 
A.      To a third party if the disclosure will enable that party to perform a business, professional or insurance function for us

B.       To an insurance institution, agent, or credit reporting agency in order to detect or prevent criminal activity, fraud or misrepresentation in connection with an insurance transaction

C.       To an insurance institution, agent, or credit reporting agency for either this agency or the entity to whom we disclose the information to perform a function in connection with an insurance transaction involving you

D.      To a medical care institution or medical professional in order to verify coverage or benefits

E.       To an insurance regulatory authority, law enforcement, or other governmental authority in order to protect our interests in preventing or prosecuting fraud, or if we believe that you have conducted illegal activities

F.       To a group policyholder for the purpose of reporting claims experience or conducting an audit of our operations or services

III. Affiliates with whom we share certain information protected by the Fair Credit Reporting Act:

A.      Unless you tell us not to, we may share information obtained from your insurance application with our affiliated companies.

B.       Our affiliated companies who may receive this information are insurance companies for obtaining a quote.
